Belts, hoses, brakes, suspension, lights, wipers, leaks. On a high-mileage car we pay extra attention to seal weep, coolant loss, and CV-axle boots.

Older rubber seals shrink and harden over time, letting oil weep past the valve covers, oil pan gasket, and rear main. High-mileage oil contains seal conditioners that help re-swell aged rubber and slow active leaks.
If the previous owner stretched oil changes, varnish and sludge have probably built up in the rings and oil galleries. High-mileage formulas carry a stronger detergent package that cleans gradually without releasing chunks.
Bearings, cam journals, and piston-to-wall clearances all widen slightly with miles. High-mileage oil holds film strength better in those wider gaps and reduces top-end noise.
Zinc and phosphorus (ZDDP) protect metal contact points under load. Higher concentration in high-mileage oil supports worn lifters, rocker arms, and timing chain tensioners.
If you have been adding a quart every few thousand miles, high-mileage oil with its thicker viscosity and conditioned seals usually reduces consumption noticeably within a few changes.
Worn lifters and lash adjusters often tick on startup until oil pressure builds. The higher film strength and faster flow of a quality high-mileage oil reduce that startup window.
